      After initial interview, parents must complete the following enrollment documents:

      • Application
      • Completed health appraisal
      • Immunization record for each child
      • Emergency cards (Child Information Cards)
      • Licensing notebook acknowledgement
      • Parent Contract

      • Parents who wish to withdraw their child from the program must give at least 2 weeks’ notice in writing. If two weeks’ notice is not given, you will be billed for the reminder of the two-week period.
